Ejaculation is a process of expulsion of semen from the male genitalia. Premature ejaculation commonly known as PE is when a man ejaculates sooner than he or his partner would like during sex. PE is also commonly referred as rapid ejaculation, premature climax or early ejaculations. Our central nervous system controls the ejaculation process. When men are sexually stimulated, signals are sent via our spinal cord to the brain. In a nutshell ejaculation has 2 main phases which are emission and expulsion.
Emission is a process when sperm travels from the testes to the prostate and mixes with seminal fluid which makes semen.
Expulsion is when the muscles at the base of the penis contract. This in result forces the semen out of the penis. In most cases, ejaculation and orgasm happens simultaneously. After ejaculation is complete, erection will slowly subside.
In our experience, PE is a problem for men with erectile dysfunction. However once erectile dysfunction has been successfully treated PE usually resolves on its own.
Although the exact causes of PE are unknown, recent studies shows that serotonin may play a role in PE. Serotonin is a natural substance that is found in our body that controls the time for ejaculation. Studies have shown that low amount of serotonin in our system shortens the time to ejaculate and leads to PE.
Other common cause is depression, stress, guilt, unrealistic expectations of sexual performance, lack of confidence and relationship problems. Your doctor will evaluate psychological problems before initiating other treatments.
It can be extremely frustrating for men when they are unable to control and ejaculate during sex or masturbation. If a man does not feel that he is in control and this affects his sex life that this may be a deciding factor to treat PE.
Usually your doctor with obtain a history that includes how often does PE happen, duration of this problem, does it happen with all partner or only one, do your reach intercourse or ejaculations happen during foreplay, and the effects of PE in your relationship are the common questions that are asked by your doctor.
There are various treatments for PE, however in our experience PE is best treated by a combination of psychological, behavioral therapy and medications. Psychological therapy may aid in calming the nerves about sexual performance and increasing your confidence and understanding to help your partners satisfaction. In addition to this we may prescribe medications via tablet or creams to delay ejaculation by reducing sensitivity and to delay ejaculation.
Behavioral therapy uses certain exercise to help build tolerance to delay ejaculation. The aim of this treatment is to help train your body to delay ejaculation as much as possible. The common methods are squeeze method. With this method, you or your partner stimulates your penis until you are close to ejaculation, during this time squeeze the base of the penis so your erection partly goes away. The goal of this therapy is for you to become aware of the sensations leading to climax. This method may help you control and delay climax. Second method is the stop start method. In this method, you/ your partner stimulates your penis until just before ejaculation. When you are about to climax, you stop until the urge to climax reduces. Once you are comfortable start stimulating your penis again. This process is usually repeated 3 times.
With the combinations of medications/ creams, behavioral therapy and psychological therapy, about 95 out of 100 men will be able to recover from PE. There will be no 100% method to promise recovery, but choosing the correct treatment and learning how to relax may help.
