General Benefits Of Circumcision

There are benefits of having circumcision:

Personal hygiene

An uncircumcised penis tends to provide an environment for pathogens/bacteria to grow. Even though boys/men were taught to clean the area underneath the foreskin, most men will not clean it each time after urinating. Thus, cleaning will be easier if circumcision is being done.

Reduce the risk of Sexually Transmitted Diseases/Infections (STD/STI)

Most studies have reported that uncircumcised men are more susceptible to STD than those who are circumcised. This is because the foreskin provides a favorable environment for bacteria/viruses/fungus to grow. However, circumcised men are still at risk of getting STD if they do not practice safe sex.

Reduce the risk of urinary tract infection (UTI)

Even though it is uncommon for a man to get a urinary tract infection. However, most studies show that UTIs are more common in uncircumcised men.

Prevent Future Penile Problems

Some men will experience recurrent balanitis which is an infection of glan of the penis. It has been shown that circumcision helps to reduce recurrent balanitis. Circumcision also can help to prevent penile issues like phimosis and paraphimosis.

Reduce the risk of penile cancer

Circumcision may even help with the reduction of penile cancer risk.

Improve sexual satisfaction

Circumcision will have the benefit of prolonging ejaculation time which may make a circumcised man can last longer in bed.

Reduced pain during sex

Studies show that circumcised men experience less pain during sex than uncircumcised men. Having foreskin put men at a higher risk of feeling pain during sexual activity due to friction and causing abrasions or tear.

Stapler Circumcision Procedure

Before the procedure, patient will have to undergo:

  • Consent taking, during which the steps below will be explained in addition to possible complications, healing period and management and wound care.
  • After obtaining the written consent, the patient will undergo measurement to decide the most suitable device size for the patient.
  • Local anesthesia will then be applied which will be done through either using topical anesthetic creams or via injection of anesthetics to block the penile nerve in case topical creams were not usable such as in case of moderate to severe tightness of the foreskin (phimosis).
  • After confirming the numbing effect of the anesthetics, areas of interest will be cleaned with antiseptic solutions.
  • An incision might be made at the foreskin to facilitate the insertion of the inner bell.
  • Once the inner bell is inserted and its correct position is confirmed to cover the glans penis it will be fixed to the foreskin by using a surgical tie.
  • The inner bell is then screwed into the outer bell and once again the correct positions of the bells are confirmed.
  • Safety pin is then removed and the stapler will be fired and held for 3 minutes to ensure clean cut of the foreskin and compression of cutaneous and superficial blood vessels.
  • Inner bell will be partially released after that to determine complete transaction of the foreskin and absolute haemostasis (absence of any bleeding points).
  • Wound is then cleaned again with antiseptic solutions and covered with antibiotic cream.
  • Penis is then bandaged using special compression bandages.
  • Patient is then discharged home with painkillers and antibiotics

Follow Ups After Stapler Circumcision

Patients will usually be given a date to come back and undergo wound inspection within 2-3 days post procedure during which the compression bandage is removed and the wound will be examined for any possible bleeding or infection. Wounds will usually be left open in most of the cases and patients will be taught on how to carry on a proper wound care in the comfort of their homes. In about 14 days after the procedure; the staples that are held together by a silicon cascade will detach.

Patients are usually advised not to practice any kind of sexual intimacy until a period of 1 month is passed.

Possible Complications From Stapler Circumcision

What to expect after stapler circumcision?

You will expect some edema and bruises after circumcision. It is normal to have some blood oozing from the edge of an incision. The recovery will take roughly 10 days. Your doctor will follow up closely until the wound is healed.

What to look out for

Before undergoing stapler circumcision; your physician will inform you of a list of possible complications, among those:

  • Bleeding: It’s not uncommon for a patient who underwent this procedure to have bleeding that ranges between small ooze to profuse bleeding from the staples site. Bleeding usually resolves with minimal intervention however in some cases it might require you to meet your physician on a short notice.
  • Edema: Following the procedure, some fluids might be trapped in the tissue surrounding the wound site as a result of undergoing inflammation after the surgical removal of the foreskin. This is a common complication and it can be avoided easily by using special dressing. Severe cases have to be managed with topical and oral medications.
  • Pain: It’s very common for patients who undergo circumcision to experience pain during the recovery period, although it’s significantly lesser in the case of stapler circumcision when it’s compared to conventional method, however, considerable numbers of patients might experience it.
  • Wound infection: Improper management and wound care might results in infection of the wound and discharge, in addition it might lead to
  • Wound dehiscence: Although undergoing circumcision with this method is considered by most of individuals who would like to avoid suturing (sutureless circumcision technique) however, in some cases the wound edges might fail to close, in this scenario your physician might decide to close the wound gaping using sutures.
  • Residual staples: The staples in our used staple device are held together with silicon ring; usually the stapler together with the ring might fall around day 10-14 post procedure. Nevertheless, some staples might remain embedded at the wound edges and will require being removed individually by your physician, and he/she needs to inform you of this possible complication prior to the procedure.

Why Undergo Stapler Circumcision

Circumcision is a surgical procedure to remove the skin covering the tip of the penis. It can be done after the newborn period but that is a more complex procedure. In Malaysia, circumcision is usually done as a religious ritual, especially for Muslim boys during their teenage years. Recently more and more men will go for circumcision for personal hygiene and preventive healthcare purposes. Circumcision does have its benefits.

Who May Go For Stapler Circumcision

  • Men who have phimosis issues need to undergo circumcision as phimosis is a medical emergency. Thus, circumcision can help prevent phimosis from happening.
  • Men with recurrent balanitis
  • Men with redundant foreskin. A redundant foreskin means that the foreskin is too long and will cause infection and inflammation easily

Overview Of Circumcision Techniques

First Generation Circumcision Technique:

A surgeon will apply local anesthesia to reduce pain and use a blade to remove the foreskin. However, this first-generation circumcision technique holds many disadvantages. For example, this circumcision technique often causes an uneven cut, increased risk of infection and the surgical wounds are larger.

Second Generation Circumcision Technique:

Second Generation circumcision techniques emerged during the middle of the 20th century. These techniques use rings, clamps, and other tools to provide a more mechanical procedure. However, this technique still has the disadvantages of bleeding, high risk of infection and the device cuts are too tight that will leave a lifelong complication.

Third Generation Circumcision Technique:

The latest technique is Stapler circumcision. This technique is faster and easier to perform. With stapler circumcision, the cut will be more precise and less complicated. There is also less bleeding and recovery time is shorter compared to the previous techniques. The recovery time is typically 10 days and the stapler will fall off by itself.

History Of Stapler Circumcision

Surgical staples have been employed for surgical procedures in the beginning of the 20th century when the Hungarian surgeon Humer Hulti came up with a prototype of a stapler. His prototype however was refined over time, and by the mid of the century different staples were produced by the Soviet Union and used for bowel and vascular anastomosis. Those were later commercialized and brought over the United States where different models have been produced for a broad range of surgical usage, including stapler circumcision.

Conclusion - Stapler Circumcision

Circumcision is an art which only fully manifests when the final result or in this case the procedure site is fully recovered. However, despite being one of the simplest and most performed procedure, it’s quite difficult to have a consistent outcome even for the most experienced surgeons, that’s because it’s quite impossible to execute a clean cut with precise suturing every single time during conventional circumcision. This is why it is important to choose a safe and effective circumcision method such as stapler circumcision.
